TRANSITIONAL HOUSING | HEARTLAND FAMILY SERVICE

Short-term transitional housing, designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community. Helps clients secure permanent housing and connects them with community-based mental health or substance abuse services and avoid unnecessary incarceration.

Staff are available 24 hours a day at the facility for resident oversight and assistance. The building consists of 15 beds that are arranged in either single- or double-occupancy bedrooms, plus bathrooms, kitchen, dining room, and living room. Residents pay 30% of their income toward their living expenses at the facility.

Staff can help clients:

- Create a plan for housing

- Connect to services to improve physical and mental health

- Support in recovery from substance use

- Support in finding employment or applying for Social Security benefits

- Connect to programs that help with life skills (cooking/cleaning, budgeting, relationship/communication skills, etc.)

TRANSITIONAL HOUSING | FRESH START

Transitional housing, providing basic needs and supportive services for homeless women without children in their custody, 19 years or older, for 24 women at a time, for up to one year.

Other services provided to clients include follow up home visits and access to basic needs for women with successful departures, in-house educational workshops, assistance with obtaining medical care and transportation, information and referral, computer learning lab, case management and advocacy, financial support, and in house recovery meetings.

RESTORATION TRANSITIONAL HOUSING PROGRAM | RESTORATION URBAN MINISTRIES

Offers faith-based transitional housing. Program participants are required to participate in religious services and classes and work with Case Manager. This is an 11-month program. Orientation occurs every August and January. Contact for detailed program requirements and information.

TRANSITIONAL SERVICES AND RISE PROGRAM | NEW LIFE TRANSITIONS OF NORTHERN ILLINOIS

Provides transitional services to formerly incarcerated individuals.

Services include, but are not limited to:
- Transitional housing
- Counseling
- Vocational assistance (including job skills training and career counseling)
- Life skills development (including budgeting, health and wellness information, parenting training, etc.)

In some cases Neuro/Bio-feedback can be provided to help those with damaged or toxic brains. This portion of the program is based on Cognitive Behavioral Therapy and can begin after a person is stabilized with Neuro/Bio-feedback.

Provides transitional housing with supportive services to those who are homeless.

The goal of transitional housing is to help individuals or couples achieve self sufficiency and increase their skills and income.

Offers comprehensive programs and housing for young women who are facing an unplanned pregnancy. Provides housing, counseling, education and career support, life skills, nutrition and health care support, Christ-centered community, referrals for other human services, and specialized education on the options of parenting or placing for adoption.

Provides support, opportunities, and transitional housing for young women transitioning out of foster care. Young women work with staff to create personalized plans to help navigate key areas of life with the goal of completing the independent living program within approximately 24 months. Staff offer guidance and connect youth with essential resources, enabling them to take the next steps toward independence. This transitional living program must be completed while the client is going to school or work, and is not an emergency shelter.

MAIN SHELTER - TRANSITIONAL HOUSING | GOD'S SHELTER OF LOVE

Open to women and their children who show commitment to being substance-free and investing in themselves long-term are eligible to stay at the 24 -hour shelter.

While at the shelter there is assistance to find employment offering resume assistance, getting signed up for long-term assistance, childcare/parenting assistance from local partners, mental health evaluation, education, and housing within 90- 120 days. The goal is for them to be set up with safe, affordable housing and the support structure needed to begin a successful step forward in their journey.

Transitional housing available to individuals experiencing homelessness and HIV. Provides up to 24 months of housing with linkage and referrals to services, depending on individual needs.

WOMEN AND CHILDREN'S SHELTER AND LIFE RECOVERY SERVICES | HOPE MINISTRIES

Offers the following services to women and women with children (these are not emergency shelter programs):

SHELTER offers short term shelter to meet the basic needs such as shelter, meals, and daily essentials, including clothing, hygiene items and laundry. Case management and referral services are provided with no fees. Please call ahead Monday- Friday between 8am-4:30pm to check on availability.

LONG-TERM LIFE RECOVERY for women (with or without children) is available to clients who are open and teachable with a willingness to change to do the hard work of life recovery. Women are offered an opportunity to work on their recovery and life skills for up to 24 months. Focus in on preparation to live independently while continuing to work on spiritual growth, recovery, basic life and work skills. Life recovery clients are required to save 75% of any income toward their transition fund.
