INDEPENDENT LIVING SERVICES | PACE CENTER FOR INDEPENDENT LIVING

Offers services which assist persons with disabilities in achieving and/or maintaining independence. Services include, but are not limited to:

- Information and Referral (personal assistant, interpreter, and housing referrals).

- Access and Advocacy (accessibility surveys, assistive devices, Braille printing).

- Skills Development (personal assistant management training, basic computer skills, public transit use, budgeting).

- Counseling (independent living, peer, vocational).

- Free Amplified Phones for Illinois residents with verifiable hearing loss who have a land line.

- Independent Living Needs Assessment.

- Mass Transit District Disability Verification.

- Reintegration into the community from nursing home, etc. (Includes possible financial assistance for moving home or into supportive living).

- PACE Presentations.

- PACE will readily provide sign language interpreters to access PACE events on request.

Provides a wide variety of independent living services for individuals with disabilities with the objective of helping them to function more independently in family and community settings and to secure and maintain appropriate employment. Services may include information and referral services, referrals for attendant care, attendant recruitment and training programs, independent living skills training, assistance in meeting housing and transportation needs, peer counseling, transition from institutions into the community, deflection from institutional placement, transition from youth to adult services, and advice and guidance regarding legal rights and benefits eligibility.

Driver's Education School is DMV certified; provides evaluation of reaction time, visual perception, and behind the wheel skills. Education and training includes classroom and on-the-road instruction. Offers adaptive equipment evaluation and training.

Durable medical equipment rent/loan at Grand Island and Scottsbluff locations. Donations of equipment accepted.

Peer mentoring, where people with disabilities support each other in their daily lives.

Housing counseling assists people with disabilities in locating, acquiring, and maintaining safe, affordable, accessible housing.

COMMUNITY LIVING, EMPLOYMENT SUPPORT SERVICES AND MENTAL HEALTH THERAPY | CANDEO

Provides HCBS Intellectual Disability and Brain Injury Waiver Community Living and employment supports to adult age 18 and up. Candeo now offers outpatient mental health therapy to children and adults. Services include:

COMMUNITY LIVING SUPPORT for individuals with disabilities including in-home and community supports that assist individuals with disabilities to live independently in the community of their choice. Serves individuals with autism, intellectual disability, developmental disabilities, brain injury, individuals dealing with interfering behaviors and dual diagnosis of intellectual disability and mental illness.

EMPLOYMENT SUPPORT for individuals with disabilities. This service provides community supports to find, obtain, learn and maintain a competitive employment position in the community. Specializes in individuals with intellectual disability, developmental disabilities, brain injuries and chronic mental illness.

HOME BASED HABILITATION SERVICES are for individuals who have a mental illness. Provides supports in the home, community and employment site to assist the individual to maintain independent living and employment.

MENTAL HEALTH THERAPY for children and adults of all ages. Candeo therapists specialize in Art and Play Therapy, Trauma recovery and more. The team also specializes in therapeutic support for adults experiencing both an intellectual disability and a mental health diagnosis.

DISABILITY RELATED SUPPORT SERVICES - LINCOLN | LEAGUE OF HUMAN DIGNITY

Services which enable persons with disabilities to acquire the skills necessary for independent living.

Barrier Removal and Information Center offers consultations to consumers, builders, businesses and organizations to increase accessibility within the home and community. Modifications to homes include redesigning living areas, and installing outside ramps and grab bars. In addition, grant assistance is offered to income- and geographically-eligible persons with disabilities.

Center for Independent Living services include information and referral, independent living skills training, financial benefits counseling, and awareness training for the general public and organizations regarding disability issues. In addition, the Center assists clients in the development and implementation of independent living plans in the following areas: home and financial management; social skills; sexuality; consumer and legal rights; peer counseling; equipment loan, repair and rental; and individual and systems advocacy.

RESIDENTIAL AND SUPPORT SERVICES | NISHNA PRODUCTIONS, INC.

Residential services include Traditional Group Homes, Community Support Program, and Supported Community Living Services.

Traditional Group Home offers training in self-help skills, household management and leisure-time skills. These programs integrate people into the community in the least restrictive environment. All individuals are encouraged to have an outside activity. Group home sites are in Red Oak, Malvern, and Shenandoah.

Community Support Program teaches independent living skills to persons living in their own apartment or home. Individuals receive additional support services as needed such as transportation, personal shopping, meal planning, socialization, self-help skills, household management skills, and money management.

Supported Community Living Program provides up to 24 hours of support to assist individuals with their daily needs.

Respite Care Program provides short-term relief in the absence of the family or caregiver, such as a vacation or a night out.

Provides comprehensive services for adults and children with intellectual or developmental disabilities. Services are designed to promote growth and support persons as they meet their individualized goals at home, work and within the community. Services include vocational, day habilitation, residential, case management, transportation, and therapeutic recreation services.

CASE COORDINATION an individually focused program that helps persons served identify, secure, and sustain resources necessary to live a normal, independent life in the community.

RESIDENTIAL SERVICES offer options to adults in a variety of living environments within the community (including hourly and 24 hour staff support in a variety of settings). The goal of all of Link's residential programs is to provide person served with experiences in independent living and social/leisure activities.
