Individuals attending CHOICE chose from a variety of activities throughout the day. Through the activities planned, individuals find out more about their community and take advantage of the resources in the area. Crafts, care of the center's flowers and plants, computer skills, preparing food, and community visits are popular activities.

Provides care and supervision in a protective setting for adults with age-related disabilities during the daytime hours. Includes transportation from and to home in Waterloo/Cedar Falls and Evansdale area.

Provides an innovative adult day health center designed to serve adults with physical or psychological challenges.

PACE (Program of All-inclusive Care for the Elderly) organization offering a complete personal health plan for eligible individuals. Supports living in the client's own home and community for as long as possible.

Participants receive all the care and services covered by Medicare and Medicaid, as authorized by the Interdisciplinary Team, as well as additional medically necessary care and services not covered by Medicare and Medicaid.

Offers a day center that serves adults with physical or psychological challenges. Available services include skilled nursing, meals, and specialized programming to meet individual needs and to assist with personal cares, while providing caregivers a break from their caregiving duties.

Nursing home facility offering adult day care, respite care, and hospice services.

Provides in-home assessments which help identify the needs and strengths of each individual and develops a client-directed care plan, that promotes independence and prevents premature institutionalization.

Resources and supportive services such as Homemaker Assistance, Adult Day Services, Emergency Home Response Services, and other programs, are determined so individuals may make informed choices regarding their own care.

Services include, comprehensive care coordination, adult day services, in-home services such as cleaning, meals, laundry, shopping/running errands, and personal hygiene, emergency home response service, and automated medication dispenser service.

- In-home services (cleaning, meals, laundry, shopping/running errands, and personal hygiene).

Provides opportunities for socialization and therapeutic activities for individuals who may be isolated in the community or who require assistance due to physical, emotional, or mental health challenges. Individualized treatment plans are tailored for clients with physical, cognitive, social, or emotional needs.

Clients are offered door-to-door transportation and buses are staffed with drivers and aides certified in First Aid and CPR. Additionally, a light breakfast and full lunch are included in the day's program. Participants can attend one day or up to five days per week, depending on their schedules and needs.

This program serves individuals with:

-- Chronic health conditions.

-- Inability to manage independently.

-- Adults who are isolated or lonely.

-- Adults with a diagnosis of Alzheimer's or Dementia.

Provides education, training, and skills development to children and adults with disabilities. Also offers opportunities for children and adults with disabilities to access, discover, learn and enjoy the great outdoors.
